To assist in completing reports correctly the first time, these instructions have been developed. They will take you screen by screen through an incident report. Each field has multiple code choices and users should review all the choices before selecting the most specific code that is still applicable to situation encountered or the action taken. The "NFIRS Quick Reference Guide" (available on the I-Drive) should be consulted if questions regarding code application not addressed in these instructions arise. As a rule, codes that end in "0" or include the word "other" in their description are poor choices and if chosen require an explanation in the narrative. Take your time filling the report out and make sure that the code chosen accurately reflects the situation. You may have to use the data in this report to testify in a court case.
A common misconception is that you need only fill out the “red” fields to completed incident report. ALL FIELDS that are color-coded in any way or include the word "Required" in the descriptive text above the entry box must be filled out to complete the report. You will need to browse through every tab to find those fields and they must contain an entry even if the applicable code is "None" or "Undetermined". Additionally, if you have information that is relevant to the incident it should be entered in the appropriate field whether or not the field is required.
The tabs on that appear on screen in FireRMS are arranged in a logical order and should be taken in sequence. If information is entered in this manner completion is much more efficient because certain data is duplicated on multiple tabs. If taken in order, this information is automatically exported to the areas where it is needed. The converse is generally not true.
It is the responsibility of the person whose name appears in the "Member Making Report" box to ensure that all required information is entered into the report and that the information, times, apparatus and responding personnel are accurately documented by the report. This person may or may not have the information required to complete all fields. None the less he/she is still expected to contact the appropriate personnel to obtain the necessary information or have that person make the necessary entries and ensure that information is complete and/or correct before indicating that report is complete on the Authorization tab. This also applies to any corrections necessary.
