EVALUATION ROUTING PROCEDURES

Secretary

  1. Start with opening up a blank evaluation form-  (G:\2-District Chiefs\AA-Dist Chiefs Shared Folder\Evaluations\*.* ).   Put the evaluation with the front page filled out (Name, S/L hours, etc.) into the officers folder on G-Drive.  Save it with a file name of- Smith John 2000- so that you will know which year it is for.

 

  1. Email the officer and DC and let them know that it is there.  The evaluation will stay in the officers folder until it is complete.

 

Officer

  1. The officer will completely fill out the evaluation go over the evaluation with the employee and let the DC know by email that it is completed.   The evaluation will stay in the officers folder.  After the DC has reviewed it and added any comments, the officer will print it out and he and the employee will sign it and send it to the Secretary for the rest of the signatures.

 

Secretary

  1. The secretary will be the last to receive the document after all signatures are on it and will file it in the employees’ permanent file.
